Thanks to Insomniac In., who informed me of the existence of HKACDD in Hong Kong, I made contact with its founder Andy during my recent visit to the city. The HKACDD comprises a group of people highly concerned with the issue of trauma and dissociation in HK. Many of you are survivors of childhood trauma yourselves, while some may be living with people with DID. With 7 million people in HK, there is a dire need for the medical profession to come to terms with Trauma and Dissociation, and recognize this condition of DID. I learned from Andy of instances of DID patients being treated as depression or schizophrenia with medications only, similar to what is happening in North America, but only worse. At least in Canada and the USA, we have a few therapists trained to recognize and treat DID.
In March 2011, when I gave a talk to the HKU Master of Social Sciences (Counseling) Alumni Association about DID. I was told the condition was unknown in Hong Kong. It is not surprising that a condition such as DID, which is not uncommon in the world, is "unknown" in HK. It is unknown because it is not recognized.
My suggestion to the members of the HKACDD is to stay close to the HKACDD, keep up the communications and self-education, and to be close to the ISSTD 國際創傷與解離學會 and its Taiwan branch, TSSTD台灣分會.
Meanwhile, think of ways of drawing the attention of psychiatrists and psychologists in HK to be recognize the conditions of trauma and various forms of disorders of dissociation. It is a very delicate matter, in trying to educate a specialist But, this is a universal problem. One must try to overcome the almost world-wide ignorance of DID. You could give your therapist a copy of publications of the ISSTD. Perhaps, not to be a self-promoter (同自已吹牛!), one could give them a copy of my book, Engaging Multiple Personalities. (It is available in HK in electronic format)
Another topic the members of HKACDD can think of is to share their experience and write about what to do with a friend, a relative, a family member, a significant loved one who is suffering from DID. If you are not the therapist, what can you do to help that person? What if the person you love suddenly becomes like a 4 year old child, crying and whimpering at the corner in bed, as an alter having a flashback of her past childhood traumatic experience? Do you leave her alone, or try to comfort her? What should you do? Would dialing 999 (the HK equivalent of US 911) and sending her to the hospital help her?
I do not claim to have all the answers. However, this must be close to home for some HKACDD members and I challenge you members to bring this out into the open and ask yourselves and the psychiatrist (who may be treating the person you love for “schizophrenia”) what can you do in such a situation.
I shall continue to visit the HKACDD website when possible, and do what I can to be helpful. I must make it very clear that I am retired and no longer practice psychiatry, so I can only make general statements and observations, rather than any specific diagnostic or treatment suggestions.

根據 TraumaAndDissociation 所言,「幼童很容易受到創傷,比成年人容易得多,當我們回頭一看,很多看似沒問題的事件,對於那個年齡來說,可能已足以構成創傷了。」(young children are very easily traumatized, much more easily than adults. When looking back, what may seem to be not such an issue might actually have been traumatizing at that age.)

原文:Psychotic-like symptoms (in the context of DID)
While they are not the primary symptoms, psychotic-like symptoms are common in dissociative disorders. This is one of the reasons why dissociative disorders are sometimes misdiagnosed as psychosis.
Hearing voices is common in DID, usually due to alters. But other psychotic-like auditory hallucinations also often occur. These voices may be 'in the head' (this is common), but may also be outside.
Visual hallucinations are also common. They can be either seeing or visualizing alters, a flashback, or genuine psychotic-like hallucinations.
Schneiderian first-rank (schizophrenia) symptoms also occur in DID, but only 8 of the 11 symptoms. The other three (thought broadcasting, audible thoughts, and delusional perception) are not part of DID.

"A tittle t trauma is any event that is beyond a person's ability to master at the time of the event. It can be something as simple as a child getting lost for a few minutes, or a child being overwhelmed with fear after a nightmare and no one comforting the child." (Schwarz, 2013, p.3)
可見,即使外表多麼簡單的一件事,例如小孩迷了幾分鐘路,或者作了個惡夢後整夜驚慌,卻沒人來照顧其弱小心靈,也可以構成創傷。

出現精神病症狀(psychotic symptoms)的成因有很多,這可以是重性精神病(psychosis,例如精神分裂症)的症狀,也可以是創傷與解離的外顯症狀(例如DID/MPD患者就很常會聽見「聲音」)。
重症精神病與創傷-解離問題的外顯症狀,表面上可能極其相似,但成因不同,便需要截然不同的治療與協助方法。例如,對於受創傷與解離症困擾的個案,醫師應鼓勵並協助個案循序漸進地與「聲音」(其實是生命的內在部分!)好好溝通,藥物治療則並非首選,用藥的份量也需要倍加留神(see Steinberg & Schnall, 2001),這一點與思覺失調、精神分裂症的介入方法分別很大。
很多有疑似幻覺的個案,在首次面談時,醫師就處方抗精神病藥(antipsychotics)作為治療方法,這過程中有足夠時間把創傷與解離症的可能性排除嗎?這樣的做法,是否值得商榷呢?如果個案只是因創傷與解離問題而出現幻覺情況,一味用抗精神病藥是否對個案好呢?這些問題值得我們好好思考!

有些個案,外顯出各式各樣的精神科診斷,例如精神分裂症(schizophrenia)、暴食症(Bulimia)、強迫症(OCD)、躁鬱症(MDP)、邊緣人格障礙(BPD)等等,其實「很可能都由不同人格所呈現」,這是因為「所有精神科診斷目前都只依外顯症狀而定,沒有排除『由不同人格呈現』一條」。

這有可能是一宗解離性神遊症(Dissociative Fugue)的個案,患者一般會不知何故地從原本的地方遠行到其他地方,同時伴隨著失憶(amnesia)等症狀,無法得知自己從何而來、怎樣到來,甚至記不起自己的過去──包括自己的身份。

5. 根據Paul F. Dell (2006),有一些類精神病症狀(psychotic-like symptoms),是可見於DID的個案上的,這也是DID常被誤診為精神病(psychosis)的原因之一;同時,所謂的「幻覺」現象,事實上可能有三種情況,以幻聽為例 (Dell, 2006):
(1)來自內在人格的聲音(hearing the voices of alter personalities)
(2)有聽覺成份的回閃(the auditory component of dissociative flashbacks)
(3)真正的精神病性的幻聽(genuinely psychotic auditory hallucinations)
我們的內心,其實由很多不同的信念、記憶、情感所組成,形成了好像很多不同的部分。正如Freud也將人格分為本我(id)、自我(ego)和超我(superego)。有時候我們內心會有矛盾,正正就是由內心不同的部分之間的衝突所造成的。這些不同的部分,彷彿就是我們的內在人格。一般情況下,這些內在人格是互相連繫、彼此分工和合作的。國際創傷與解離學會 (International Society for the Study of Trauma and Dissociation, ISSTD) 台灣分會「台灣創傷與解離學會」 (Taiwan Society for the Study of Trauma and Dissociation, TSSTD) 創會人暨心靈之美心理治療所院長張艾如(Irene Ai-Ju Chang)臨床心理師(我們都稱她為張老師), 依其12年解離個案的評估與治療經驗而提出、多年來於國內外學術會議中發表且大力向民眾推廣的「葡萄理論」(The Grapes Theory) 身-心-靈健康模式,正正就是在演繹心靈智慧與內在世界之實況了!

Since you mention the autobiographic book by Robert Oxnam, I like to bring out something that may help those living with MPD or DID to see a ray of optimism.
Dissociation is not always pathological. A surgeon in the middle of a nasty divorce must be able to dissociate so that he can concentrate on the operation and not on the breakup of his family. He must be able to rely on his ability to dissociate.
In a person with MPD, one may afford an alter to completely concentrate on some specific pursuit of learning. Robert Oxnam is multi-talented. He is an academic, an accomplished scholar, and for ten years working as the president of Asian Society. He was consultant to two US presidents…….. He led financial-cultural tours of China for Bill Gates, Warren Buffett and former US present George H.W. Bush. One of the alters is fluent in Chinese so that he could teach in the prestigious Beijing University. One alter also learned to play the cello. I doubt if he could have achieved so much if he had been a “normal” person with one identity.
So having multiplicity is not necessarily all bad. The most important requirement is that the parts do not fight against each other. So learn to cooperate and learn to love and value each other! This is what a therapist will help you to achieve, but before you can find such a therapist, begin by learning to improve communication among the alters. 5. 根據Paul F. Dell (2006),有一些類精神病症狀(psychotic-like symptoms),是可見於DID的個案上的,這也是DID常被誤診為精神病(psychosis)的原因之一;同時,所謂的「幻覺」現象,事實上可能有三種情況,以幻聽為例 (Dell, 2006):
(1)來自內在人格的聲音(hearing the voices of alter personalities)
(2)有聽覺成份的回閃(the auditory component of dissociative flashbacks)
(3)真正的精神病性的幻聽(genuinely psychotic auditory hallucinations)
